Program Scope Includes
- Cross-agency referral workflows with local clinics & nonprofits
- Policy & systemic barrier advocacy to improve resource access
- Community coalition participation representing SDOH priorities
- Partner training on whole-person, dignified case management
Service Availability
Available to residents of Fresno County, including the city of Fresno and surrounding communities. Services are free or sliding-scale with no insurance requirement.

Monday – Friday: 8:00 AM – 5:00 PM
Closed on federal holidays
