LumiThrive Health is a Fresno County-wide community health navigation resource. We help residents throughout the county connect with food, housing, transportation, health coverage, and mental health support, and we coordinate help close to wherever you live.
How We Help in Fresno County
LumiThrive Health brings the same whole-person approach to Fresno County. As a community health navigation and social determinants of health (SDOH) case management organization, we connect people to resources rather than providing direct medical care. Every program below is available to residents of Fresno County (CA).
Coordinated County-Wide Support
We do not maintain a physical office in every community. Our team coordinates from our Fresno office and works with local partners across Fresno County to reach residents where they are.
